New Patient Information at Proliance Center

Preparing for Your Initial Visit

We kindly ask all new patients to arrive 15 minutes early for their first appointment to ensure a smooth and stress-free check-in process. Prior to your visit, you should complete the new patient questionnaire provided by our office staff. Completing these forms in advance allows your clinician to review important background information and focus fully on your care during the appointment.

If you need to cancel or reschedule your appointment, please notify our office at least 24 hours in advance. This courtesy allows us to offer the time to another patient who may be in need of care.

Telehealth and In-Office Appointments

Seeing Dr. Mike (Telehealth Appointments)

Dr. Mike currently sees all patients via secure telehealth. Telehealth arrangements are coordinated by our office staff at the time your appointment is scheduled. By the day of your visit, all required intake forms should already be completed. At your scheduled appointment time, Dr. Mike will contact you directly for your session. Telehealth allows for convenience, privacy, and continuity of care while maintaining the same high clinical standards.

Seeing Other Proliance Clinicians (In-Office Visits)

If you are seeing one of our other professionals, your appointment will take place in person at our office. Patients enter through Suite 250 and will be welcomed into a shared waiting area. Our team will be notified of your arrival, and your clinician will greet you at your scheduled time. As with all appointments, we ask for at least 24 hours’ notice for cancellations.

What You Can Expect at Proliance Center

A Comfortable and Welcoming Environment

Proliance Center was intentionally designed by Dr. Mike to help patients feel relaxed, safe, and at ease. Adult counseling rooms offer the warmth and comfort of a living room, while child and adolescent spaces resemble a welcoming playroom environment. These thoughtful design elements help reduce anxiety and create a setting that supports openness, trust, and effective treatment.

Flexibility, Availability, and Personalized Care

At Proliance Center, no concern is too small or too complex. While appointments are typically scheduled during regular business hours, our clinicians are available to support existing patients during urgent situations when needed. Our approach emphasizes accessibility, continuity, and a strong therapeutic relationship built on trust, respect, and collaboration.

Language Options

All Proliance clinicians are fully fluent in English, and some, including Dr. Mike, are also fluent in Spanish. This allows patients to communicate comfortably in the language that feels most natural when discussing personal or sensitive matters.

Our Payment Policy

Proliance Center operates as a fee-for-service, private-pay medical group, allowing us to deliver a higher level of personalized, thoughtful care. By not participating directly with insurance companies, Dr. Mike and our clinicians are able to make treatment decisions based solely on professional judgment and the individual needs of each patient—without outside restrictions.

Payment is due at the time of service and can be made directly to the doctor or at checkout with our administrative staff. While we do not accept insurance payments or referrals, we are considered a non-preferred provider, and we provide detailed receipts (superbills) that patients may submit to their insurance company for possible out-of-network reimbursement.
